When I come back from an illness of 3 days or less (with or without a doctor's note), do I have to sign a form saying that yes indeed I was sick and specify the nature or symptoms of the illness?

Depending on the policies for employees of the organization, you may be required to sign a form stating that you were absent due to illness. However, the very specific details of your illness are not necessary for employers to collect in order to process your sick-leave. Employers may collect, use and disclose personal employee information without consent, as long as it is only personal information that is reasonably needed to manage their work relationship. That still means that employers must limit their collection to only what is necessary to manage their employees.
