How can I get a copy of my medical records if I do not know where my Doctor is? I wrote a letter and it was returned to me a week later.

The mandate of the Information and Privacy Commissioner is outlined in the Health Information Act (the HIA) which provides individuals with the right to request access to health information in the custody and control of custodians (physicians, hospitals, long-term care centers, etc.) while ensuring that the privacy of individuals is protected.

The mandate of this Office does not include assisting individuals in locating their physicians.
