Can I request a copy of my health records?
The Health Information Act (HIA) provides you with the right to request access to your health records if they are held by a custodian. There are many different types of health information custodians. Custodians include:
- A health service provider paid in part or in whole by the Alberta Health Care Insurance Plan to provide health services
- Regional Health Authorities
- Hospitals
- Nursing Homes
- Community health councils
- Subsidiary health corporations
- Alberta Health and Wellness and the Minister of Health and Wellness
- Any pharmacist or licensed pharmacy
If your health information is held by a custodian, you have the right to request access to it by making a formal request under the Health Information Act.
